This form must be completely filled out and submitted by any company or gaming group requesting complimentary GM badges. Badges will not be printed for names without correct account numbers. The deadline for all badge requests is June 22nd,  2008. Any requests received after that date may not be processed before the convention. Don't forget to indicate how you want to pick up your badges: as a single packet or have each GM come and get their own badge.  If you have any questions, please email or call us at 206-957-3811.
